What Happened
- KKR acquired Envision for $9.9 billion in 2018 using $7 billion in borrowed money — one of the biggest debt-fueled PE deals since the financial crisis, with patients unknowingly backstopping the bet
- Envision's 'secret sauce' was keeping ER doctors deliberately out-of-network so they could surprise-bill patients at inflated rates — even when the hospital itself was in-network
- Filed Chapter 11 bankruptcy in May 2023 and wiped out $7 billion in debt — KKR lost its $3.5 billion equity stake while the company was split into two pieces with new owners
- Class action settlements in Texas, California, and Arizona forced Envision to write off inflated charges and refund patients who were overcharged for out-of-network ER and anesthesia services
The Damage Done
- Patients routinely received two bills for a single ER visit — one from the in-network hospital, and a surprise bill from an Envision doctor they never chose and couldn't avoid
- The No Surprises Act of 2022 specifically targeted the billing model Envision pioneered — but by then, millions of families had already been hit with bills they couldn't fight
- Envision's 30 million annual patient encounters meant the surprise billing machine operated at industrial scale — if you visited an ER, there was a real chance an Envision doc was the one billing you
- After bankruptcy, Envision split into two companies under new ownership — but the physicians, patients, and communities that absorbed years of cost-cutting got nothing back
